OT Kids Plus, LLC will provide occupational and physical therapy services to students in order to maintain compliance with the students’ IEP. The schools in which these students are placed do not provide these therapy services and also servicing students who are severely disabled and home-bound students.(This resolution is responsive to Part 200 requirements to provide a free and appropriate public education (FAPE) to all students. If the nature or severity of the student’s disability is such that it precludes them from attending an in-district program, then a program must then be provided to the homebound student.
ASSESSMENT PROCESS:
Informal communication is ongoing between parents, provider and other school personnel. This communication is in the form of written letters and consults with teachers. Formal communication is via end-of-the-year summaries, monthly progress and statistical reports. Assessment of progress will be done at CSE and Annual Review Meetings.

Ladies and Gentlemen:
WHEREAS the Board of Education wishes to obtain the services of a Contractor to provide Occupational Therapy and Physical Therapy services as recommended by the Committee on Special Education, and
WHEREAS the Board has selected OT Kids Plus, LLC. to provide these services.
NOW THEREFORE BE IT RESOLVED: That the Board of Education hereby authorizes the Superintendent of Schools to enter into a contract with the above-named provider in the amount of $115,240.00. |
