Scope: Set Apart will provide Physical Therapy services to students in order to maintain compliance with the students IEP. (This resolution is responsive to Part 200 requirements to provide a free and appropriate public education (FAPE) to all students. If the nature or severity of the student's disability is such that it precludes them form attending an in-district program, then a program must then be provided to the homebound student. |

|
WHEREAS the Board of Education wishes to obtain these services from Set Apart Physical Therapy
WHEREAS the Board of Education has selected Set Apart Physical Therapy to provide these services,
NOW THEREFORE BE IT RESOLVED: That the Board of Education hereby authorizes the Superintendent of Schools to enter into a contract with the above named provider in the amount not to exceed $12,000. |
