Resolution:

WHEREAS the Board of Education wishes to enter into a contract with Accucare Nursing & Home Care provide a 1:1 nurse for an Out of District Student. The nurse will accompany the student on the bus to and from school and will also accompany the student during the school day.
WHEREAS the district administration has designed in conjunction with Accucare Nursing & Home Care a program which meets the objectives set forth by the Board and the New York State Education Department, and,
WHEREAS the Board of Education authorizes the Superintendent of Schools to enter into an agreement with Accucare Nursing & Home Care to provide these services, and
NOW THEREFORE BE IT RESOLVED: That the Board of Education hereby authorizes the Superintendent of Schools to enter into an agreement with Accucare Nursing & Home Care at a cost not to exceed $132,344.00.
