Amount: $14,650.00
Scope: Agreement with Interactive Health to amend existing contract #2022-00000699. This amendment will extend the contract until August 2022. This amendment represents an increase of $14,650 to provide additional services. The total contract will be $32,650.00 Interactive Health, LLC will provide services pertaining to the implementation of the My Brother’s Keeper Family and Community Engagement Program (MBK FCEP) Grant. Interactive Health, LLC will provide the district team with consultation related to the re-evaluation of the district-wide Family and Community Engagement Policy. Interactive Health, LLC will also work with the grant management team on assessment activities.

Resolution:

WHEREAS the Board of Education wishes to amend contract# 2022-00000699 with Interactive Health, LLC in the amount of $14,650.00 to continue consulting services to provide the district team with consultation related to the re-evaluation of the district-wide Family and Community Engagement Policy. Interactive Health, LLC will also work with the grant management team on assessment activities for the 2021-2022 school year,
WHEREAS the district administration has designed in conjunction with Interactive Health LLC a program which meets the objectives set forth by the Board and the New York State Education Department, and,
WHEREAS the Board of Education authorizes the Superintendent of Schools to amend contract# 2022-00000699 with Interactive Health, LLC to provide these services, and
NOW THEREFORE BE IT RESOLVED: That the Board of Education hereby authorizes the Superintendent of Schools to amend contract #2022-00000699 with Interactive Health, LLC at a cost not to exceed $32,650.00.
It is acknowledged and agreed that pursuant to the terms of that certain intermunicipal agreement by and between the City of Yonkers Board of Education (“BOE”) and the City of Yonkers (the “City”) as filed in the Office of the City Clerk on June 16, 2014, the terms of this agreement are subject to review and revision by the Corporation Counsel, as well as approval by the City’s Board of Contract and Supply. Furthermore, this Agreement shall not be enforceable until signed by both parties and approved by the Corporation Counsel’s Office.
