1. Agreement with Westchester Institute for Human Development

Amount: $ 5,000
Scope - Comprehensive assistive technology evaluations will be done by the Westchester Institute for Human Development assistive technology team. The comprehensive evaluation may require the services of one or more of the following disciplines:
Speech -Language Pathologists (Augmentative/Alternate Communication/Instructional Technology); and/or Occupational Therapists (Access/Instructional Evaluation fee will be determined as follows:
One discipline: $1,380
Two disciplines $1,890
Travel cost per school visit $25 under 20 miles
$50 over 20 miles
There will be a $225 charge if an evaluation has to be re-written in medical or educational format.
Training can include : observation of student in school activities: adult and peer interaction: meeting with team members (parent, paid professionals, teachers and related service providers); and hands-on training sessions with devices. Training fees are as follows:
$225.00 per hour or $560 for 3 contracted hours. Travel cost per school visit is $25 up to 20 miles and anything over 20 miles is $50.

WHEREAS the Board of Education of the City of Yonkers is required to provide services to the students of the Yonkers Public School District, and
WHEREAS these services will be provided by Westchester Institute for Human Development, and
NOW THEREFORE BE IT RESOLVED: That the Board of Education hereby authorizes the Superintendent of Schools to enter into a agreement with the Westchester Institute for Human Development in the amount not to exceed $ 5,000.
