Scope: This amendment to agreement is to provide additional funds for OT and PT Evaluations and Triennial Evaluations. Judy A. Snyder Physical Therapist, PC will provide OT/PT services during the 2024 - 2025 school year in the Yonkers School District. Evaluated students have been identified by the CSE team as needing evaluations to determine services needed.

Resolution:

WHEREAS the Board of Education wishes to amend its existing contract with Judy A Snyder to continue to provide these services to District Students,
WHEREAS the Board has determined that these services can best be provided by Judy A. Snyder Physical Therapist, PC,
NOW THEREFORE BE IT RESOLVED: That the Board of Education hereby authorizes the Superintendent of Schools to enter into a contract with Judy A. Snyder Physical Therapist, PC in a total amount not to exceed $278,000.00.
