|
CO3 to annual contract 2021-‘0139 “Maintenance and Repair of Oil Burners and Cleaning and Repair of Boilers,” to Ultimate Power, Inc., for $152,263.50 increasing the contract from $1,580,846.00 to $1,733109.50 with no change in term.
The subject contract was awarded via Resolution 15.13 dated July 15, 2020. It was amended by Change Order 1 via RES 23.22 dated April21, 2021 to add $148,846.00 in funds increasing the contract to $864,846.00. It was extended with no increase in funding by Change Order 2 via RES 16.7 dated June 16, 2021.
School 14 proposed $47,827.20 for the reconstruction of the collapsed fire box under boiler No 2.
School 31 proposed $104,436.00 Emergency Declaration reconstruction of the flood damaged heating plant, including the installation of new burners, condensate pump system, controls and wiring.
The above figures are estimates. Final amounts are to be determined by time and material as per the contract. |

Resolution
|
Ladies and Gentlemen;
WHEREAS: via Resolution 15.13 dated July 15, 2020 the Board authorized the award of contract 2021-‘0139 “Maintenance and Repair of Oil Burners & Boilers,” to Ultimate Power, Inc., in an amount not to exceed $716,000.00 for a term of 365 days renewable for 2 additional years, and
WHEREAS: via Resolution 23.22 dated April21, 2021 the Board authorized Change Order 1 to add $148,846.00 in funds increasing the contract to $864,846.00 with no change in term, and
WHEREAS: via Resolution 16.7 dated June 16, 2021 the Board authorized Change Order 2 to extend the contract by one year under the same terms and conditions increasing the contract amount by $716,000.00 to $1,580,846.00 and the term by 365 to 730 calendar days, and
WHEREAS: additional work is required to maintain the operation of the heat plants at School 14 and School 31 for an upset cost of $152,263.50 increasing the contract from $1,580,846.00 to $1,733,109.50 with no change in term.
NOW THEREFORE BE IT RESOLVED: That the Superintendent of Schools is hereby authorized to issue Change Order 3 to contract 2021-‘0139 “Maintenance and Repair of Oil Burners & Boilers,” to Ultimate Power, Inc., 45 Nancy Street, West Babylon, NY 11704 in the amount of $152,263.50 increasing the contract from $1,580,846.00 to $1,733109.50 with no change in term. |
