CHANGE ORDER 1 for CAPITAL Construction Contract at Rosemarie Ann Siragusa School Modular Building roof project to Milcon Construction Corp. for and additional $19,140.01 and 180 days. By Resolution 17.23 dated May 9, 2021 the Board of Trustees authorized the award of Contract 2021-0804 IFB 6606 “Group A Building Envelope Restoration” for CIP’s 10862 Rosemarie Ann Siragusa School Modular, 10863 School 23 and 10866 Riverside High School to Milcon Construction Corp. for $3,406,670.00 and a term of 365 calendar days. Each project listed has a specific PO number. This is a change order for 10862 Rosemarie Ann Siragusa School Modular Building PO 2021-6735 and the covering contract 2021-0804. This change order is required for unforeseen conditions (or field conditions) and owner directed extra work including but not limited to: 1. Replacement of roughly 200 feet of cast iron storm sewer and rebuild three drainage structures in the parking lot serving the roof leaders. Includes hand excavation in places and restoration of the pavement. 2. Repairs to downspout on main building. 3. Storm sewer cleaning and inspections. After several credits and application of the $15,000.00 contingency the change order amount required is $19,140.01. This increases PO 2021-6735 from $259,695.00 to $278,835.01 and the cover contract 2021-0804 from $3,406,670.00 to $3,425,810.01. The term of each is extended by 180 days for paperwork and closeout documents. |

Ladies and Gentlemen: WHEREAS by Resolution 17.23 dated May 19, 2021 the Board of Trustees authorized the award of Contract 2021-0804 IFB 6606 “Group A Building Envelope Restoration” for CIP’s 10862 Rosemarie Ann Siragusa School Modular Building, 10863 School 23 and 10866 Riverside High School to Milcon Construction Corp. for $3,406,670.00 and a term of 365 calendar days WHEREAS: in the course of executing the contracted work it was determined that additional work including storm sewer replacement is required in the amount $19,140.01, and WHEREAS: This increases PO 2021-6735 from $259,695.00 to $278,835.01 and the cover contract 2021-0804 from $3,406,670.00 to $3,425,810.01. The term of each is extended by 180 days for paperwork and closeout documents. NOW THEREFORE BE IT RESOLVED: That the Superintendent of Schools is hereby authorized to issue Change Order 1 to contract 2021-0804 IFB 6606 “Group A Building Envelope Restoration” for CIP’s 10862 Rosemarie Ann Siragusa School Modular Building, 10863 School 23 and 10866 Riverside High School and to Purchase Order PO 2021-6735 for CIP 10862 “Group A Building Envelope Restoration” at Rosemarie Ann Siragusa School Modular Building to Milcon Construction Corp, 142 Dale Street, W. Babylon, NY 11704 for the amount of $19,140.01; increasing the contract amount from $259,695.00 to $278,835.01 and extending the contract for an additional 180 days to 545 calendar days. |
