Rationale:
|
This is to authorize the issuance of the Change Order 5 on Contract 2021-0139 “Maintenance and Repair of Oil Burners and Boilers” with Ultimate Power, Inc. in the amount of $385,660.00, increasing the contract amount from $2,567,969.50 to $2,953,629.50 with no change in the contract term.
This is owner directed additional work using the unit bid prices in the original contract.
Work entailed in this change order includes but is not limited to:
Two new expansion tanks at Ella Fitzgerald Academy for $35,560.00.
Section replacements at Boilers One and Two at Paideia School 15 for $130,284.00.
New boiler feed tank, vacuum condensate pumps & Boiler Three section replacement at the PEARLS Hawthorne School for $219,816.00.

Ladies and Gentlemen:
WHEREAS: By Resolution 15.13 dated July 15, 2020 Contract 2021-0139, “Maintenance and Repair of Oil Burners and Boilers” was awarded to Ultimate Power, Inc. for $716,000.00 and a term of 365 calendar days, and
WHEREAS: By Resolution 23.22 dated April 21, 2021 Change Order 1 was authorized for additional work at Charles E. Gorton High School, School 22 and School 30 increasing the contract by $148,846.00 to $864,846.00 with no change in term.
WHEREAS: By Resolution 16.7 dated June 16, 2021 Change Order 2 was authorized to extend this contract for one year increasing the contract amount by $716,000.00 to $1,580,846.00 and the contract term to 730 calendar days.
WHEREAS: By Resolution 14.9 dated November 17, 2021 Change Order 3 was authorized for additional work at Rosemarie Ann Siragusa School and Montessori School 31 increasing the contract by $152,263.50 to $1,733,109.50 with no change in term
WHEREAS: By Resolution 20.26 dated June 14, 2022 Change Order 4 was authorized for additional work at Kahlil Gibran School for $118,860.00 and the second annual extension increasing the contract amount by $834,860.00 to $2,567,969.50 and the term to 1,095 calendar days.
WHEREAS: This is to authorize the issuance of Change Order 5 on Contract 2021-0139 for additional owner-directed work at Ella Fitzgerald Academy, Paideia School 15 and PEARLS Hawthorne School for $385,660.00 increasing the contract amount from $2,567,969.50 to $2,953,629.50 with no change in the contract term.
NOW THEREFORE BE IT RESOLVED: That the Superintendent of Schools is hereby authorized to issue Change Order 5 to Contract 2021-0139 “Maintenance and Repair of Oil Burners & Boilers,” to Ultimate Power, Inc., 45 Nancy Street, West Babylon, NY 11704 in the amount of $385,660.00 increasing the contract from $2,567,969.50 to $2,953,629.50 with no change in term.
